<p>Utah Healthcare Corps - Salt Lake County CHW</p> <p>Patient navigators (PNs) use their life experiences and knowledge of their communities to reduce barriers to healthcare and build capacity through resource connection, health education, and health system navigation. With over 60 years of evidence-backed interventions across multiple health domains, CHWs play an integral role in addressing healthcare access and providing a holistic healthcare approach. Further help on this page can be found by clicking here.</p> <p>Member Duties : Helping individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information Helping to build understanding and social capital to support healthier behaviors and lifestyle choices Helping health care and social service systems become culturally relevant and responsive to their service population Providing informal counseling, support and follow-up Improving access to health care services and information through resources, educational program, activities and referrals.
