Confers with nursing leadership and/or the nurse(s) on the next shift to provide continuity of care and relay information to other nursing shifts to assure proper follow-up of physician's orders and instructions.</p> </li><li> <p>Makes routine rounds of assigned residents and modifies standard nursing procedures for care, as needed.</p> </li><li> <p>Ensures that medications are administered within required time frames and as needed (PRN). Documents any adverse reactions and takes appropriate measures.</p> </li><li> <p>Inventories medication supplies and orders medications from the pharmacy as needed.
