Your best is waiting to be discovered.</p> <p>Day - 10 Hour (United States of America)</p> <p>This is a Stanford Health Care Tri-Valley job.</p> <p>A Brief Overview</p> <p>The Wound Ostomy Continence (WOC) Nurse is responsible for patient care and education related to acute and chronic wounds, pressure injuries, ostomies, and fecal and urinary incontinence.</p> <p>Locations</p> <p>Stanford Health Care Tri-Valley</p> <p>What you will do</p> <ul> <li>Assesses patients' pertinent prior medical histories, skin integrity problems, acute and chronic wounds, and ostomies to develop an appropriate plan of care for each patient. </li><li>Educates patients and families on wound and problems with skin integrity etiology, systemic factors which they can impact, and self-care where practical. </li><li>Provides real-time education to all staff members to improve their wound and ostomy care skills. </li><li>Consults with physicians, nurses, case managers, pharmacists and therapists to provide coordinated care specific to each patient. </li><li>Documents clearly the plan of care, education, specific steps in care, including products and how they are to be used. </li><li>Participate in two PI Surveys annually </li><li>Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. C-I-CARE is the foundation of Stanford Healthcare - Tri-Valley's patient-experience and represents a framework for patient-centered interactions </li></ul> <p>Education Qualifications</p> <ul> <li>Graduate of an accredited School of Nursing Required </li><li>Graduate of, or completion of, a WOC Nursing Education program accredited by the WOCN or has successfully completed experiential program and certification through the WOCN Board. Required </li></ul> <p>Experience Qualifications</p> <ul> <li>1+ year to 2 years of recent WOC Nursing experience.
