<p>Job Description</p> <p>Position: Prevention Services Coordinator - HeyDenver Program</p> <p>Classification: Non-Exempt</p> <p>Reports to: Prevention Services Manager</p> <p>Salary Rate:</p> <p>Level 1 (Entry level up to 2 years of direct experience): $23.11 per hour</p> <p>Level II (3+ years of direct experience): $25.19 per hour</p> <p>Bilingual premium for qualifying languages is 3.5% higher than starting salary if pass bilingual exam on first week of hire.</p> <p>About Us</p> <p>At Colorado Health Network (CHN), we believe that our employees are our most valuable asset. If so, we would love to hear from you!</p> <p><strong><strong><strong><strong><strong><strong><strong><strong><strong>____</strong></strong></strong></strong></strong></strong></strong></strong></strong></p> <p>Why Work at CHN?</p> <p>As a full-time employee, you will enjoy a competitive benefits package, which includes:</p> <p>Health & Wellness Benefits: Health, Dental, Vision, Life, Short- and Long-Term Disability Insurance, and an Employee Assistance Program (EAP)</p> <p>Employee only share of health insurance premium is 100% paid for by CHN</p> <p>Paid Time Off (PTO): Generous leave policy (at the end of one year full time employee accrue 3 weeks of PTO), 13 paid holidays, up to 2 floating holidays, and 1 day of wellness time (all time off benefits for part time employees are prorated based on the number of hours scheduled per week).</p> <p>Retirement Savings: 403(b) plan with employer matching $1 for $1 up to 3% and $0.50 on the $1 for 4 and 5%</p> <p>Education & Professional Development: Tuition reimbursement and access to ongoing learning opportunities</p> <p>Additional Benefits: Flexible Spending Accounts (FSA), Health Spending Account (H SA), Voluntary, EAP</p> <p>Eligible for Federal Loan Forgiveness Program</p> <p>Position Summary:</p> <ul> <li>Facilitation of HIV/HCV/STI testing and risk reduction counseling services </li><li>Coordination and implementation of the PrEP support program </li><li>Coordination and implementation of community outreach and education </li></ul> <p>Essential Job Functions:</p> <ul> <li>Administers HIV, hepatitis C (HCV), and STI testing and individualized risk reduction counseling to PWUD (People Who Use Drugs), as well as people living with HIV/AIDS; members of the LGBTQ+ community; people experiencing or at risk for homelessness; and other populations who may benefit from educational programs and services related to sexual health and harm reduction. </li><li>Coordinates and promotes the PrEP support program through outreach to current clients and the community. Coordinates screenings, intakes, and client navigation for PrEP access and retention. </li><li>Facilitates educational presentations and marketing in the community, including individual and group level interventions, outreach events, and other materials. </li><li>Represents the organization at community events and collaborative meetings as needed. </li><li>Maintains current understanding of overdose, HIV/HCV/STI prevention issues and trends; participates in additional trainings when offered. </li><li>Maintains and develops referral network of support and prevention services and provides linkage to care (LTC) as needed. </li><li>Works collaboratively with the Prevention Services team and statewide prevention teams to develop and support programs that are culturally competent and responsive to clients accessing services at HeyDenver. </li><li>Facilitates intern and volunteer training and support in the areas of HIV/HCV/STI testing, outreach, and other prevention services. </li><li>Maintains and develops referral network of HIV support and prevention services. </li><li>Participates in departmental and agency retreats and meetings. </li><li>Works days and evenings as needed, some evenings and weekends required. </li><li>Provides in person support during the operation of the CHN syringe access program including, but not limited to, safe disposal; distribution of syringes and safe injection/snorting supplies; overdose education; data entry; and intakes. </li><li>Provides in person guidance, training, and support to volunteers and fellow staff. </li><li>Must have current and valid Colorado driver's license; must be able to pass driving record check per CHN insurance requirements. </li><li>Must be able to be insured by CHN driving and liability insurance companies. </li><li>Must have personal insurance on own vehicle if driving on behalf of CHN. </li><li>Must be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities. </li><li>Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues.
