<p>Job Summary</p> <p>Provides assistance to Case Managers in assessing the needs of clients, arranges and coordinates services/resources County, State and Federal programs.</p> <p>Essential Functions</p> <ul> <li>Completes eligibility assessment of clients; assists Case Managers in developing a care plan to meet the service needs of the client; arranges and coordinates needed services for clients; and provides follow-up with clients as needed. </li><li>Maintains Federal, State, and County mandated client records; prepares reports; and writes letters and correspondence. </li></ul> <p>Contacts and advocates with other community agencies on clients' behalf.</p> <ul> <li>Assists in crisis interventions. </li><li>Provides information and referral for the public and agencies; receives, processes, and tracks referrals. </li><li>Completes telephone intakes; develops and maintains CIRTS client tracking systems for active clients; develops the approach on scheduling and completing client assessments; and develops and maintains information and referral database. </li></ul> <p>Represents divisional interests at community meetings; attends seminars and in-service training.</p> <ul> <li>Tracks all client contacts of contact on behalf of client. </li><li>Provides interviews to walk-in clients. </li><li>Review and implements new and changing program requirements. </li></ul> <p>Minimum Qualifications</p> <ul> <li>High school diploma or GED required. </li><li>One (1) year of related experience. </li><li>Fingerprinting required. </li></ul> <p>LICENSES/CERTIFICATES:</p> <ul> <li>Must possess and maintain a valid Florida Driver's License with any applicable endorsement(s) to drive a County vehicle as provided in CMA 5805. </li></ul> <p>Supplemental information</p> <ul> <li>Salary offers above the minimum pay grade may be considered based on qualifications. </li><li>Essential Employees may be required to work during an undeclared or declared emergency.
