We are driven by Positivity which inspires us to be our best and to go above and beyond for our patients, for one another, and for our community.</p> <p>If you're ready for a rewarding new career, join Team Bronson and be part of the experience.</p> <p>Location</p> <p>BBC Bronson Battle Creek, BMH Bronson Methodist Hospital</p> <p>Title</p> <p>APP Advanced Illness Management - Palliative Care NP or PA-C</p> <p>Bronson Methodist Hospital in Kalamazoo, MI is seeking to employ an inpatient Palliative Care Nurse Practitioner or Physician Assistant part-time.</p> <p>Position Highlights:</p> <ul> <li>The inpatient Advanced Illness Management (AIM) Nurse Practitioner or Physician Assistant will provide palliative care to adults with advanced, life-limiting progressive and terminal illness. </li><li>Candidate will provide initial and ongoing clinical assessment for patients with advanced and/or progressive medical conditions who are consulted for palliative care. </li><li>The NP or PA-C will collaborate with the AIM team (social work, chaplain, AIM physician), inpatient attending physician, primary care provider, specialist, patient and their families to effectively manage the palliative care needs of the patient. </li><li>Will be responsible for assessing the patient's needs, establishing goals of care as well as recommending treatments and therapies that are consistent with the patient's goals and the need to manage their symptoms. </li><li>The main areas of focus are: symptom management, assistance with decision-making/Advanced Care Planning; collaboration with the interdisciplinary team to provide and facilitate emotional, spiritual, bereavement support and coordination of care until death or discharge. </li><li>Coverage will be at Bronson Methodist Hospital (Kalamazoo county) and Bronson Battle Creek (Calhoun county) </li></ul> <p>Qualifications:</p> <ul> <li>Successful completion of an accredited Physician Assistant or Nurse Practitioner Program </li><li>Ideal candidate will have 3-5 years of recent clinical experience related to the area of practice required </li><li>Current unrestricted or pending Licensure in the state of Michigan </li><li>Strong communication and interpersonal skills </li><li>Commitment to providing compassionate and quality patient-centered care </li></ul> <p>Bronson Methodist Hospital offers:</p> <ul> <li>A supportive and collaborative work environment </li><li>Competitive compensation with Annual Quality Incentives </li><li>Comprehensive benefits package </li><li>Relocation Assistance, CME Stipend and Sign On Bonus </li></ul> <p>About Kalamazoo, Michigan: Located halfway between Chicago and Detroit, we occupy a beautiful part of the state of Michigan that offers cultural opportunities, economic diversification and very affordable real estate. Interprets and correlates subjective and objective patient data, formulating a diagnosis. Under the direction of the physician, reviews patient findings and develops a treatment plan within the scope of practice. Performs above activities in the outpatient and inpatient settings. Employees providing direct patient care must demonstrate competencies specific to the population served.</p> <p>Successful completion of a Physician Assistant program</p> <p>Licensure as Physician Assistant and state DEA license to prescribe medications</p> <p>Current prescriptive authority and collaborative agreement with oversight physician(s) should also be in place</p> <p>Must maintain all requirements for medical staff privileges</p> <ul> <li>Ability to utilize word processing, spreadsheet, presentation programs, and other software relevant to the job </li><li>Regularly communicates clearly and effectively both verbally and in writing (Utilizing SBAR technique) </li><li>Communicates with internal customers such as, physicians, multi-disciplinary team, finance, management and external customers such as, patients, third party payers, community agencies on a daily basis and often simultaneously </li></ul> <p>Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time.</p> <p>The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects.</p> <ul> <li>Concurrent Clinical Management </li><li>Coordinates care with the multi-disciplinary team to facilitate implementation of patients' progress towards outcomes, which includes discharge or transfer of care. Acts as a clinical resource for the multi-disciplinary team. This is accomplished by: </li></ul> <p>o Assertive coordination among the multi-disciplinary team</p> <p>o Identifying common patient complications and utilizes appropriate preventative measures.</p> <p>o Assisting in the development and integration of quality measures/evidence into clinical practice.</p> <p>Utilization Review:</p> <ul> <li>Performs an initial comprehensive assessment of the patient to determine if the patient requires a higher intensity of service and requires transfer to a hospital setting.
