<ul> <li>Provides clinical assessment, service coordination, therapeutic interventions, and discharge planning for the residential program. </li><li>Assessment of individuals with substance use disorders with treatment recommendations provided to treatment team and other service providers. </li><li>Collaborates with internal and external treatment providers to ensure appropriate treatment recommendations and discharges plans are in place. </li><li>Conducting individual and family sessions, group therapeutic activities, and discharge planning, as needed, for adults who present for service. </li><li>Responsible for accurate and timely documentation of all information in the clinical record. </li><li>Actively respond to individuals who show signs of crisis, which may include mental health symptomology or unsafe behaviors. Work as a team with other staff to de-escalate. </li><li>Assists with screening referrals for appropriateness and coordinating admissions </li><li>Communicate admission to agency staff, programs and / or community partners who are currently involved with patient. </li><li>Develop a behavioral treatment plan with measurable objectives that the individual can work toward while completing residential treatment. </li><li>Complete chart audits while updating and monitoring accuracy of clinician tracker. </li></ul> <p>Qualifications</p> <ul> <li>Bachelor's degree or higher in human services field. </li><li>Minimum one (1) year experience working with individuals in behavioral health. </li><li>Ability to pass a level II Background clearance and drug test. </li><li>Valid FL drivers' license, insurance, and safe driving record. </li><li>Ability to lift and move objects weighing up to 25 pounds </li><li>Stamina to stand, walk, sit, and perform tasks for extended periods. </li><li>Manual dexterity and visual acuity for documentation. </li><li>Auditory acuity for verbal interactions whether face-to-face or over the phone. </li><li>Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures. </li><li>Ability to use a computer </li><li>Attention to detail </li><li>Ability to work independently and as part of a team, in collaboration with other community partners. </li><li>Ability to manage stressful situations and display appropriate work demeanor and boundaries. </li><li>Knowledge of community resources and ability to effectively communicate with community organizations. </li><li>Ability to demonstrate excellent customer service. </li><li>Strong oral and written communication skills. </li></ul>
