<p>Job Description:</p> <p>Physical Therapist (Job ID 607)</p> <p>Birth to Three/Early Intervention</p> <p>Annual Calendar/Waterbury</p> <p>Part Time/Per Diem</p> <p>Major Responsibilities:</p> <ul> <li>Home visitation with infants and toddlers and their families </li><li>Conduct developmental screenings and/or eligibility evaluations </li><li>Assist families with the implementation of functional, age-appropriate strategies </li><li>Develop Individualized Family Service Plans (IFSP) with families </li><li>Provide service coordination to families </li><li>Assist child with transition to preschool programming </li><li>Work in transdisciplinary team of Early Childhood professionals </li><li>Position requires home visitation and regional travel throughout Waterbury </li><li>Regular and consistent attendance is considered an essential function of this position </li></ul> <p>The Physical Therapist works under the supervision of the Birth to Three Director in the division of Early Childhood & Family Programs.</p> <p>Qualifications:</p> <ul> <li>CT DPH Licensed/Certified Physical Therapist </li><li>Experience with Infants, Toddlers and Families </li><li>Service Coordination training (or willingness to obtain) </li><li>Excellent organizational skills and computer literacy </li><li>Driver's license and auto insurance required </li></ul> <p>Application Procedure:</p> <p>Applications should be submitted for consideration by visiting EdAdvance's employment page at, http://edadvance.org/about-us/employment, and click on the APPLY button next to the job posting of interest.</p> <p>Human Resources</p> <p>EdAdvance</p> <p>P.O.
