<p>Employment Type:</p> <p>Part time</p> <p>Shift:</p> <p>Day Shift</p> <p>Description:</p> <p>Per Diem Social Worker - Hospice Albany</p> <p>The Community Hospice has a per diem opportunity for a social worker to join our hospice care team caring for patients in the Capital region as part of our Interdisciplinary Team in homes and Nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Per diem Monday- Friday 8a-4:30p. Days are flexible.</p> <p>Responsibilities:</p> <ul> <li>Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care. </li><li>Provide specialized skills and mental health therapy for clients. </li><li>Identify barriers which impact optimal patient functioning. </li><li>Ensure patients receive appropriate resources in most appropriate setting. </li></ul> <p>What you will need:</p> <ul> <li>NYS SW Licensure. </li><li>Valid NYS Drivers license. </li><li>Minimum 2-3 years experience ideally caring for seniors or those with advance illness. </li><li>The Community Hospice provides a thorough orientation and a supportive work environment. </li></ul> <p>Apply today for more information!</p> <p>Pay Range: $30.00 - $43.08</p> <p>Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
