And above all, it means exceptional care, simply delivered - and we couldn't do it without you.</p> <p>Make a Difference</p> <p>The Testing/Outreach Specialist will provide HIV related Counseling, Testing and Referral services. The Testing/Outreach Specialist will conduct targeted outreach to high-risk HIV negative individuals. The Specialist will link individuals to HIV treatment, health information, education, referrals, risk reduction interventions, condom distribution, and STI awareness, testing and treatment.</p> <p>Your Exceptional Skills and Qualifications</p> <ul> <li>Provide HIV Counseling, Testing and Referrals to at-risk populations in compliance with the standards put forth by the Indiana State Department of Health </li><li>Conduct targeted outreach to at-risk populations </li><li>Engage at-risk populations in their own neighborhoods or venues where they typically congregate to provide HIV related health information, education, referrals and linkage to care services </li><li>Link individuals to medical care for HIV infection and other support services </li><li>Maintain effective communication with Community Infectious Disease (CIDC) medical case managers </li><li>Assist Ryan White Grant Manager in growing CIDC prevention programs </li><li>Work at both East and North locations </li><li>Complete data collection, entry and reporting </li><li>Provide HIV positive test results </li><li>Other duties as assigned </li><li>Bachelor's Degree or equivalent experience </li><li>Successfully complete the CDC/ISDH HIV Tester training </li><li>Equipped to work with multicultural and diverse populations </li><li>Self-motivated and willing to work flexible shifts and flexible locations </li><li>Ability to maintain confidential and sensitive information </li><li>Fluency in Spanish a plus </li></ul> <p>Apply today for immediate consideration.</p> <p>Why Community?</p> <p>At Community Health Network, we build teams that deliver exceptional care through empathy, communication and collaboration.
