In this dynamic role, you'll provide direct care to patients and support to families, while also serving as a floating clinician-temporarily assigned to different hospice branches based on census needs or staffing gaps. If you're energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.</p> <p>As a Hospice RN - Regional Support, You Will:</p> <ul> <li>Deliver high-quality, compassionate nursing care for hospice patients </li><li>Manage and implement individualized plans of care </li><li>Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) </li><li>Assess and address patients' physical, emotional, spiritual, and environmental needs </li><li>Accurately document all care provided and ensure compliance with hospice protocols </li><li>Educate families and caregivers on care instructions, medications, and safety </li><li>Participate in the on-call rotation and provide after-hours care as needed </li><li>Collaborate with physicians, referral sources, and the care team </li><li>Assist with referrals, regulatory documentation, and payer compliance </li><li>Travel to various Gentiva hospice locations as assigned to fill urgent needs </li></ul> <p>Schedule & Travel:</p> <p>This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities.
