<p>There is full-time PACT RN position open in the Primary Care Division at the Milwaukee VA. This is only open to current Milwaukee or CBOC employees.</p> <p>This position supports the Primary Care Patient Aligned Care Team (PACT) Model. The Registered Nurse (RN) Care Manager demonstrates leadership in delivering and improving holistic care through collaborative strategies utilizing the PACT model of care. The RN works in partnership with an assigned panel of patients and team members for effective disease and chronic care management.</p> <p>The RN Care Manager executes position responsibilities of the PACT model by using the nursing process and evidence-based practice to collaborate with services internal and external to the VA to facilitate care transition to effectively meet the patients' needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
