<p>POSITION SUMMARY:</p> <p>The Student Success Coordinator is a member of the community school team for Catholic Charities. Manager will work primarily with an identified set of youth to actively support the scholars and ensure that a strong commitment to academic, attendance and social achievement is evident.</p> <p>The Coordinator will also focus on items related to College and Career Readiness objectives during the daytime programming. This includes the completion of Student Success Plans, Hartford Public Schools high school applications and Choice applications as well as facilitating college exploration activities. The position facilitates leadership development for middle school students. It is a 35 hour per week position that works both during the school day and in after school.</p> <p>JOB RESPONSIBILITIES:</p> <ul> <li>Collaborate with school partners to identify at-risk scholars </li><li>Facilitate care team meetings called "All-Star meetings" for each scholar on the caseload to discuss and develop a care plan that focuses on academics, attendance and social achievement </li><li>Provides guidance in the area of academic, behavioral and social-emotional prevention and intervention </li><li>Work with students and guardians to ensure all students complete their high school choice applications </li><li>Assist students in completing their Student Success Plans </li><li>Provide career awareness education in the classrooms </li><li>Secure professionals to visit classrooms on a regular basis to expose students to career options </li><li>Coordinate high school visits and high school fair visits for students and parents/guardians </li><li>Coordinate college visits for middle school students to expose them to college campuses </li><li>Facilitate youth leadership development through Student Ambassadors, Serve-Up community services, and others as well as summer opportunities in the Milner Leads program </li><li>Help students prepare their first resume listing their extracurricular and community service work that will be a starting point in creating their portfolio for their college applications </li><li>Enter data into a web-based client management system to track progress </li><li>Assist in the operation of after school programming </li><li>Provides supervision and training to after school staff. </li><li>Maintain regular communication with parents and school staff as assigned. </li><li>Participate in school committees as appropriate. </li><li>Monitor programmatic implementation of after school programming as assigned. </li><li>Participate in the development and coordinate school vacation and summer programming. </li><li>Assist in ensuring regular student feedback is obtained for ongoing improvement. </li><li>Other duties as required. </li><li>PQI Functions: </li><li>Ensure the consistent accurate collection of the data </li><li>Identify patterns and trends for program </li><li>Use results of data to inform supervisor of trends that may impact services </li></ul> <p>EDUCATION AND EXPERIENCE:</p> <ul> <li>Bachelor's degree preferred with three years experience working with children and youths in an elementary school. </li><li>At least three years experience in coordinating after school programming in an urban setting </li><li>At least two years of supervisory experience. </li></ul> <p>QUALIFICATION:</p> <ul> <li>Ability to develop, coordinate and facilitate after school programs for elementary youth. </li><li>Strong interpersonal, relationship-building, verbal and written communication skills </li><li>Knowledge of public school systems helpful. </li><li>Computer literate in Microsoft Word environment and ability to manage data. </li><li>Bilingual desired (Spanish) </li><li>Experience with either Cayen or ETO data systems </li><li>Experience in the field of instruction. </li><li>Classroom management and/or group facilitation skills </li><li>Able to work with urban children and youth </li><li>Experience working in schools in summer or after school programming preferred </li></ul>
