<p>Description</p> <p>POSITION SUMMARY</p> <p>The Intensive Care Coordination Supervisor is responsible for oversight of services for youth who are assigned to the Intensive Care Coordination Tier of service within Pathways to Success. Responsible for the selection, training and supervision of Intensive Care Coordinators. Provides clinical consultation with program staff regarding clients as needed concerning program services. Assures compliance with HFS Care Coordination and Support Organization requirements and all regulations in which the services and agency are provided under.</p> <p>ESSENTIAL FUNCTIONS AND RESPONSIBILITIES</p> <ul> <li>In coordination with Manager of Care Coordination, interviews, selects, trains and supervises Wraparound Care Coordinators. Monitors employee performance and completes quarterly and annual evaluations. Works to resolve any performance issues or concerns. </li><li>Provides oversight to client referrals, intake and admissions, assuring timely response to referral sources. </li><li>Maintains positive relationships and open communication with community providers and others with whom the agency collaborates on behalf of clients. </li><li>Oversees record keeping and reporting processes to insure that flow of written communication is current and effective. </li><li>Provides leadership to the Utilization Review committee to ensure that the process is completed and that results are used to improve the quality of services. </li><li>Participates in the all required committees including but not limited to monthly staff meeting and leadership meeting. </li><li>Responsible for all program reviews and the success of the programs key performance indicators. </li><li>Ensure needs of clients are met utilizing Systems of Care values. Ensure staff have a variety of skills and resources to utilize when working with youth and families. </li><li>Monitors direct service requirements of staff to ensure targets are being met and monitors caseload size to ensure accurate ratio. </li><li>Adheres to and follows all other required duties necessary and assigned by Manager. </li></ul> <p>Requirements</p> <p>QUALIFICATION AND EXPERIENCE</p> <ul> <li>Strong organizational and time management skills. </li><li>Excellent oral and written communication skills. </li><li>Excellent interpersonal skills. </li><li>Must possess the experience, personal qualities and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services consistent with Systems of Care and Wraparound. </li><li>Ability to supervise and support staff both onsite and remote locations. </li><li>Must meet DCFS/CANTS Clearance Requirements </li><li>Must be at least 21 years of age </li><li>Must possess and maintain a valid driver's license and maintain proof of automobile insurance </li></ul> <p>EDUCATION AND/OR EXPERIENCE:</p> <ul> <li>A Master's Degree in Social Work, Counseling, or Psychology from an accredited program, or in a related human service field. License of Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC) preferred. </li><li>Supervisory experience in the human services field is required. </li><li>Knowledge of community programs, services and evidence-based practices and a variety of treatment modalities used to work with youth and their family to navigate child serving systems. </li></ul> <p>CARE & TCI</p> <p>Hoyleton has adopted the CARE (Children and Residential Experience), developed by Cornell University, to guide our culture and practice. CARE is grounded in six core principles: Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed.</p> <p>Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire. Ongoing application of these principles is expected and essential to successful job performance.</p> <p>TCI (Therapeutic Crisis Intervention). Employees must:</p> <ul> <li>Pass the written test annually </li><li>Pass the physical test every six months (if applicable to their role) </li></ul> <p>Consistent participation and application of both CARE and TCI principles drive company culture and are essential to job performance.</p>
