The Admission Coordinator assists in the coordination all aspects of MDS and RAI processes by supporting the MDS Coordinator and Social Worker. Investigates, assesses, and initiates interventions to help Elder's cope with social, emotional, economic, and environmental challenges associated with living in a LTC facility.</p> <p>Reports to: Nursing Home Administrator</p> <p>FLSA: Non-Exempt</p> <p>Job Requirements</p> <p>Minimum Education</p> <p>Associate's Degree required</p> <p>Bachelor's Degree preferred</p> <p>Required Licenses: RN license required</p> <p>Other Certifications: BLS required</p> <p>Required Skills</p> <ul> <li>Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. </li><li>Must be able to work in a stressful environment and take appropriate action. </li><li>Demonstrates proficiency in verbal and written communication skills </li><li>Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external </li></ul> <p>Essential Functions</p> <p>Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.</p> <p>Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.</p> <p>Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.</p> <p>Performs Medicaid screenings as indicated for NHP.</p> <p>Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities.</p> <p>Committed to the development of the staff, Elders, and families as collaborative team members.
