This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.</p> <p>Qualifications</p> <p>Qualifications:</p> <ul> <li>Licensed Vocational Nurse, per the state requirements. </li><li>Good communication skills, assessment skills, and strong clinical documentation skills. </li><li>Ability to read, write, and speak the English language. </li><li>Reliable transportation. Valid and current auto liability insurance. </li></ul> <p>Essential functions:</p> <ul> <li>Under the direction of the RN, assist in identifying the patient's physical, psychosocial, and environmental needs. </li><li>Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. </li><li>Provide care utilizing infection control measures that protect both staff and patient (OSHA). </li><li>Records treatments administered in medical record. </li><li>Records clinical documentation according to accepted regulatory, corporate and professional guidelines. </li><li>Participates in weekly case conferences. </li><li>Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress. </li><li>Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment. </li><li>Participates in in-service training programs. </li><li>Responsible for assuring patient safety. </li><li>Complies with state regulations and national standards </li><li>All documentation will be completed and synched by 9:00AM the following day, 7 days per week. </li></ul> <p>We are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.</p> <p>Diversity, Equity, and Inclusion are at the heart of Cantex.
