<p>As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes.</p> <p>Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.</p> <p>Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.</p> <p>Records daily progress in EMR.</p> <p>Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.</p> <p>Communicates/conferences with primary nurse and other disciplines.</p> <p>Consults with physician regarding client needs and plan of care.</p> <p>Minimum Education Requirement: High School diploma or GED - Successful completion of a CT state approved LPN certificate program</p> <p>Minimum Licensure, Certification, Registration - Current and valid LPN license - Valid CT driver's license & a good driving history</p>
