We recognize that a person's life experiences can provide firsthand knowledge relevant to being successful in the work that we do.</p> <p>PSH Program Mission</p> <p>The Permanent Supportive Housing (PSH) program is a housing first initiative which emphasizes moving high acuity youth experiencing homelessness into permanent housing while also addressing their current needs through an integrated team composed of case managers, a peer support specialist, and a therapist. This team provides services to ensure the youth, ages 18-24, maintain their housing as well as helping them work towards goals they create in relation to improving their mental and physical health, vocational or educational pursuits, building life skills, and connecting to the community.</p> <p>Position Summary</p> <p>To provide consistent, individualized, and thorough case management services to clients who have moved out of homelessness and are living in housing throughout Salt Lake County. The case manager will support the clients in obtaining and maintaining stable housing and addressing current needs as defined by the client. The PSH case manager position is housed out of the VOA Youth Resource Center and includes mobile work meeting with clients out in the community/in their homes.</p> <p>Essential Duties</p> <ul> <li>Provide case management services to clients by assisting them in developing goals and a case management plan including but not limited to obtaining and maintain housing, medical care, mental health care, state and government benefits, and assistance with legal issues. </li><li>Attend staffing/clinical staffing meetings three times weekly; participate and present client information at said meetings to achieve greater client success through collaboration as part of a multi-disciplinary team. </li><li>Assist clients in locating, applying for, and securing housing after they have been triaged for the program through the coordinated entry process </li><li>Assess (and regularly reassess) client needs utilizing appropriate, agency approved tools and identify priority needs to coordinate services as needed. </li><li>Interact with all clients in a professional, trauma-informed manner while utilizing techniques such as motivational interviewing, Progressive Engagement, and appropriate boundary setting. </li><li>Collaborate with program manager in ensuring program fidelity, documentation accuracy, confidentiality, and intra-agency collaborations. </li><li>Develop client files including case management plans, referrals, and/or discharge plans as appropriate utilizing a trauma-informed lens and with a client first focus. </li><li>Utilize both internal tracking sheets and required case note web infrastructure in a timely and accurate manner. </li><li>When appropriate, work with clients' families to promote client self-sufficiency and continuity of care. </li><li>Actively engage with clients by demonstrating cultural responsiveness and sensitivity. </li><li>Be on call 3 times a year for the Youth Resource Center </li><li>Demonstrate a proactive willingness to learn and an openness to critical feedback. </li><li>Attend work on a punctual, regular, and predictable basis. </li><li>Utilize agency vehicles as needed to provide transportation to clients. </li><li>Assist in moving clients in and out of housing. </li><li>Perform other functions as necessary or assigned. </li></ul> <p>Requirements</p> <p>Qualifications</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p> <p>A degree in the field of social services and two years' experience working in the social services field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:</p> <ul> <li>Must be able to qualify and pass Utah Department of Human Services Case Management Certification within 60 days of being hired. </li><li>Exhibits clear communication skills and cooperates with others. </li><li>Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public. </li><li>Ability to utilize Trauma Informed Care and Housing First practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision when necessary. </li><li>Willingness to accept supervision and direction. </li><li>Ability to move around the facility and interact with clients for extended periods. </li><li>Must be at least 21 years of age, possess a current driver's license, have a good driving record and be insurable on the agency liability policy. </li><li>Pass Utah DHS - Office of Licensing background screening for those working with vulnerable populations. </li><li>Proficiency in Microsoft Office. </li></ul> <p>Physical Demands</p> <ul> <li>Ability to lift and carry 50 lbs. </li><li>Ability to move around the facility, interacting with clients for extended periods. </li><li>Work is generally performed in an office environment.
