This role works with a multidisciplinary team to ensure efficient, high-quality care for these patients throughout their treatment journey. The Nurse Navigator will contact patients, collaborate with providers, and work with the team to remove barriers, improve outcomes, and enhance satisfaction. Acting as a clinician, advocate, educator, and inpatient facilitator, they oversee patient care and serve as a resource for chronic disease management. The role requires knowledge of patient development and care appropriate to the patient's age.
