This role works with a multidisciplinary team to ensure efficient, high-quality care for these patients throughout their treatment journey. The Nurse Navigator will contact patients, collaborate with providers, and work with the team to remove barriers, improve outcomes, and enhance satisfaction. Acting as a clinician, advocate, educator, and inpatient facilitator, they oversee patient care and serve as a resource for chronic disease management. The role requires knowledge of patient development and care appropriate to the patient's age. They will use evidence-based practices and understand care pathways for these patients.</p> <p>Required: Certifications/Licenses/Experience:</p> <ul> <li>Active Registered Nurse (RN) in Indiana or other compact licensure state </li><li>Hospital experience with quality assessment and/or quality improvement experience will be considered in lieu of the education requirements </li></ul> <p>Preferred Certification/License/Experience:</p> <ul> <li>BSN </li><li>Minimum of 3 years' relevant experience in caring for AMI/CABG/HF/COPD patient populations </li><li>Advanced certification in cardiovascular nursing </li><li>White Belt Certification or equivalent per Quality </li></ul> <p>Other Key Words: Performance // Cardiac Nursing</p> <p>Department: Performance Improvement</p> <p>Hours: Part-time, 24 hours/week</p>
