Rising Ground is headquartered in downtown Brooklyn.</p> <p>Job Summary:</p> <p>The Social Work Supervisor oversees the day-to-day case management activities for the assigned NSP and LSP programs, including Aftercare. The Social Work Supervisor oversees the assessment, planning, family engagement, and evaluation process for case planners, ensuring compliance with all guidelines, quality assurance standards, and performance indicators for Close to Home programs. Work includes guiding referrals and discharge activities, coordinating and facilitating the treatment teams, monitoring, and guiding referrals and discharge activities as well as coordination of linkages with community-based support programs and services, and overseeing aftercare current aftercare case planners. The Social Work Supervisor manages all social service needs and ensures that all ACS, OCFS and the Courts' mandated timeframes are met for all the youth supported within the programs. This person must provide quality services in the areas of leadership, supervision, case reviews and monitoring, progress note reviews, and advocacy for our clients in placement in order to meet and exceed expectations with respect to program goals, quality services, compliance, and best practices. The Social Work Supervisor will also assist with intakes for all youth coming into CTH NSP's and LSP.</p> <p>Job Responsibilities:</p> <ul> <li>Provide effective ongoing supervision feedback to Case Planners who report to Social Work Supervisor. </li><li>Maintain documentation of supervision through the completion of supervisory logs, and Connections Supervisory Notes for NSP and LSP cases. </li><li>Regularly review all progress notes, court documentation, and other relevant documentation to ensure full compliance and best practices in services provided to NSP and LSP youth. </li><li>Develop and maintain a case planning team in each NSP and LSP facility who works in collaboration with all milieu staff. </li><li>Set high expectations and enforce strict deadlines for staff to ensure excellence in all areas. </li><li>Monitor all components of Treatment Team meetings including challenges, goals, YLS assessments, treatment plan tool and PAMS compliance. </li><li>Attend transition meetings, Service Plan Review meetings, staff meetings, admin meetings, treatment team meetings, red flag meetings and discharge meetings. </li><li>Support the principles of each facility's Evidence Based Model with the end goal teaching the young people to make positive, lasting changes in their behavior by preparing them to be self-sufficient through education and work experience. </li><li>Collaborate with agency departments to meet program goals and objectives including the HR, Training and QI Departments in the areas of performance management, interviewing, mandated training, professional development, PAMS Score card, Evolve needs, and QI goals. </li><li>Oversight, monitoring, and approval of FASP reports in Connections, Transition plans, and all Federal mandates. </li><li>Ensure that records are accurate and complete in compliance with city, state, & agency regulations </li><li>Collaborate with Director of Community Based Programs to ensure referrals are submitted to aftercare for continuity of services for all youth in placement transitioning back to their community. </li><li>Complete Intakes for youth entering and transferring to NSP's, and South Ozone LSP. </li><li>Cover for Divisional Director when needed. </li><li>In the absence of case planner, cover case planner workload as needed. </li><li>Other related task that may be assigned </li></ul> <p>Qualifications</p> <ul> <li>Master's degree in human services or related field required / LMSW preferred. </li></ul> <p>§ Minimum of two (2) years of experience, with at least one (1) year of experience, in a supervisory capacity in the field of child welfare, or a minimum of three (3) years of experience, with at least one (1) year of experience in the field of juvenile justice, under qualified supervision.</p> <ul> <li>Experience with casework documentation in CNNX </li><li>Excellent writing and organizational skills </li><li>Experience with juvenile justice system involved youth preferred </li><li>Proficient with Microsoft Office suite </li><li>Commitment to progressive social justice and anti-racist practices </li><li>Belief in an individual's capacity to change. </li><li>Bi-lingual (Spanish) a plus </li></ul> <p>Supervisory Responsibility</p> <p>Social work supervisor Supervisor will supervise 2 case Planner from Ozone Queens Male Limited Secure Facility, and 1 Case planner from Bensonhurst Brooklyn Male Non-Secure Placement</p> <p>Work Environment:</p> <p>The Social Work Supervisor will work from Ozone Queens facility 2 days a week and Bensonhurst Brooklyn facility 2 days a week.</p> <p>One remote workday per week is permitted upon Divisional Director Approval.</p> <p>Position Type/Expected Hours of Work:</p> <p>This is a full-time position of 35 hours per week. Daily schedule and hours will fluctuate to meet the needs of the team and clients. The Social Work Supervisor will be responsible for maintaining their own schedule appropriate to the needs of the team and youth served, which include both during the day, evenings, and weekends on occasion. The Social Work Supervisor will be expected to participate in facility treatment team meetings, family nights, parent nights and other events to help to engage youth in placement and streamline services. The schedule requires flexibility to meet the participation expectations. The Social Work Supervisor will be available to respond to program needs after hours or weekend on-call by phone.</p> <p>Travel (if required):</p> <p>The Social Work Supervisor will provide their own transportation and work-related travel expenses outside of the facilities will be reimbursed upon review.
