<p>Overview</p> <p>The Department of Psychiatry's Moms in Recovery Program seeks a social worker or experienced case manager to join a dynamic team focused on providing integrated addiction treatment to pregnant and parenting people. This role is at our Addiction Treatment Program in the Rivermill office complex in Lebanon, NH.</p> <p>As a Resource Specialist, you will have the opportunity to work with patients and their children in a clinic setting. You will be an essential part of an interdisciplinary team that works collaboratively to support people with their health and recovery goals.</p> <p>We seek candidates with robust organizational, interpersonal, and communication skills. Case management and prior clinical experience is preferred. More information about the program can be found here: https://www.dartmouth-hitchcock.org/moms-recovery</p> <p>This is a full-time, 40-hour/week position, day-shift position.</p> <p>Responsibilities</p> <p>Acts as an advocate on behalf of patients/families in obtaining needed services and/or resources for which they are eligible. Updates information resource information directory and maintains related literature & applications for distribution to staff and families in specific area of concentration. Documents services provided in patient's medical record and applicable databases. Negotiates and advocates access for patients to selected skilled, acute or long term care facilities. Identifies available transportation resources and negotiates with patient/family for cost effective, safe transportation. Works with patient/family to assess patient eligibility for medication, co-pay and additional resource assistance. Facilitates the patients/families application to public and private government and community agencies, primarily Medicaid. Provides information and assistance to patients/families to secure financial and health assistance, organization financial assistance, and other governmental programs. Performs other duties as required or assigned.</p> <p>Qualifications</p> <ul> <li>Bachelor's degree in Social Work or Human Service field or the equivalent in education and experience required. </li><li>Excellent interpersonal, communication, and organizational skills required. </li></ul> <p>Required Licensure/Certifications</p> <ul> <li>BLS within 30 days </li></ul>
