<p>COMMUNITY HEALTH WORKER I -II</p> <p>Full-time employment contingent upon federal funds availability.</p> <p>GENERAL SUMMARY</p> <p>The purpose of this job is to provide community-based home visitation and provide an access point for high-risk pregnant and parenting residents to receive knowledge, support, and referral to community services that will support family success and build self-sufficiency. The position will work with high-risk families who need extra support navigating community resources and focusing on child development.</p> <p>ESSENTIAL DUTIES & RESPONSIBILITIES</p> <ul> <li>Coordinates care and support services for families in high-risk neighborhoods to determine needs. </li><li>Records, collates, and documents information for appropriate research and reporting. </li><li>Manages and updates client records in appropriate County systems and in accordance with program protocols and requirements. </li><li>Conducts risk assessments, develops care plans and makes appropriate referrals. </li><li>Completes home visits and monitors family health and safety evaluations. </li><li>Develops training materials and classes to educate families on available programs and resources; educates and shares information via various channels such as one-on-one education, videos, paper materials, and calls. </li><li>Responds to case inquiries and program and benefit questions, providing appropriate information and resources. </li><li>Liaises between clients, families, medical, mental health, and social service agencies. </li><li>Engages in community outreach and recruitment of eligible program participants, including creating and distributing promotional materials; promotes resources and programs to families, businesses, schools, and organizations. </li><li>Perform related work as required </li></ul> <p>MINIMUM QUALIFICATIONS - Community Health Worker I- Pay Scale 104</p> <p>Required Education and Experience</p> <ul> <li>At least two years of relevant college-level course work or an Associate's degree from an accredited college or university in Human Services, Education, Social Work, or a related field </li><li>At least 1-3 years of related work experience in social work or serving diverse populations in crisis </li><li>Any equivalent combination of training, education, and experience that provides the required skills, knowledge, and abilities </li></ul> <p>MINIMUM QUALIFICATIONS - Community Health Worker II-Pay Scale 106</p> <p>Required Education and Experience</p> <ul> <li>Bachelor's degree from an accredited college or university in Human Services, Education, Social Work, or a related field </li><li>At least 1-3 years of related work experience in social work or serving diverse populations in crisis </li><li>Any equivalent combination of training, education, and experience that provides the required skills, knowledge, and abilities. </li><li>VETERANS are encouraged to apply: Please provide your joint services transcript with your application. </li></ul> <p>Pay Scale-CHW-I-104</p> <p>Pay Scale-CHW-II-106</p> <p>Monday-Friday 8:00 a.m.-5:00 p.m.</p> <p>Position : 11003003</p> <p>Code : 25118-1</p> <p>Type : INTERNAL & EXTERNAL</p> <p>Job Family : PUBLIC HEALTH & WELFARE</p> <p>Posting Start : 08/15/2025</p> <p>Posting End : 09/01/2025</p> <p>Details : Benefits Information</p> <p>MINIMUM HOURLY RATE: $22.30</p>
