Proof of license(s) certifications, and/or education must be submitted with each application.</p> <p>Examples of Duties</p> <p>Under general supervision, plans, coordinates, and provides professional public health assessment and care coordination for screening and diagnostic services related to cancer testing for individuals and providers in Baltimore County.</p> <p>Essential Job Duties</p> <ul> <li>Provides nursing assessment, care planning andreferral services to individuals, and institutions. </li><li>Reviews clinical history and demographic information for clients upon assignment of cases. </li><li>Contacts assigned clients/representatives in accordance with state and county policies. </li><li>Coordinates with clients and providers to ensure appropriate and timely screening and follow up. </li><li>Maintains professional relationships with providers. </li><li>Ensures provider acknowledgement and compliance with state clinical guidelines when necessary. </li><li>Provides case management, ongoing evaluation, and education of client care plan. </li><li>Maintains accurate and detailed documentation in electronic health record and program databases </li><li>Independently organizes and prioritizes workload to meet program deadlines and performance metrics. </li><li>Assesses client need for community-based resources in accordance with the social determinants of health. </li><li>Ensures compliance with state, federal and Medicaid regulations. </li><li>Provides health screening education and counseling in the areas of diseaseprevention, health maintenance, and mental health. </li><li>Participates in the instruction and orientation of new public health nurses. </li><li>Prepares and maintains nursing records and reports. </li></ul> <p>Examples of Other Duties</p> <ul> <li>Performs other related duties as required. </li></ul> <p>Qualifications</p> <p>Required Qualifications</p> <p>Graduation from an accredited college or university with a bachelor's degree in nursing.</p> <p>OR</p> <p>Graduation from an accredited diploma school of nursing;</p> <p>Plus</p> <p>At least one year of professional nursing experience. (Additional qualifying education may be substituted on a year-for-year basis for the required experience.)</p> <p>OR</p> <p>Graduation from an accredited college or university with an associate in arts degree in nursing:</p> <p>Plus</p> <p>At least two years' professional nursing experience.</p> <p>Additional qualifying education may be substituted on a year-for-year basis for the required experience.</p> <p>Knowledge, Skills, and Abilities</p> <ul> <li>Candidates should demonstrate: </li><li>Experience working in cancer screening and prevention </li><li>Strong case management and care coordination skills. </li><li>Excellent documentation skills </li><li>Experience with client education and coordination. </li><li>Experience connecting clients to community resources </li><li>Ability to work effectively with diverse populations and utilize translation resources. </li><li>Experience working with multidisciplinary teams. </li><li>Ability to communicate professionally with medical providers and community partners. </li><li>Excellent customer service and interpersonal communication skills </li><li>Strong self-organization, time management, and independent scheduling abilities. </li><li>Ability to manage multiple priorities and meet deadlines independently </li><li>Proficiency in Microsoft and experience working in data platforms. </li><li>Ability and willingness to participate in community outreach events, including occasional evening or weekend work. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Experience in public health or community-based programs. </li><li>Experience working with grant-funded programs and performance metrics. </li></ul> <p>Licenses and Certificates</p> <p>Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.</p> <p>Proof ofLicenses, Certifications and Education</p> <p>Applicants are required to submit proof of education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable. Duties may expose incumbents to various bio-medical hazards.</p> <p>Medical Examination and Employment Background Investigation</p> <p>Applicants selected for employment with Baltimore County must successfully complete a physical examination, drug screening and employment investigation, including, but not limited to criminal background, education certification and fingerprinting checks.</p>
