<p>This position is for a full-time advanced practice provider with our team at Corewell Health Lakeland Hospitals Cardiothoracic - St. Joseph.</p> <p>Essential Functions:</p> <ul> <li>Ability to formulate an individualized plan of care indicated by the patient's response to treatment, and evaluate overall plan for effectiveness. </li><li>Collaborates with the physician in order to plan and coordinate patient care. </li><li>Perform complete screening exams and routine exams in order to determine a baseline, reflect the nature of the patient visit, diagnose. </li><li>Arranges for ordered diagnostic, surgical and therapeutic services.
