<p>Position Title: Licensed Vocational Nurse/Licensed Psychiatric Technician</p> <p>Department: Nursing</p> <p>Reports To: Director of Nursing Services or Charge Nurse and Program Director</p> <p>POSITION SUMMARY</p> <p>The LVN/LPT is primarily responsible for providing direct patient care services. Job duties include medication management, medication administration, counseling and health maintenance. Evaluates patient needs and assists in implementing plans to meet those needs, aids in formulation of the treatment plan, monitors patient's medication intake and treatments, as directed, assists in maintaining and ordering adequate medical supplies, charts into the medical/legal records, reports and records observations that reflect the nursing care given and the patient's progress, educates patients on the medical aspects of treatment, interviews, screens and performs health assessments per established protocols, monitors and reports any significant changes in patients condition or behavior to Charge Nurse.</p> <p>This position requires providing service to one or more age populations including adolescents and geriatric patients in a manner that demonstrates an understanding of the functional/developmental age of the individual served. In addition, nurses shall have the ability to assess, treat, and appropriately interact and care for individuals with mental health and substance use disorders. This is a field-based position and clearance to drive by TTC's insurance broker is required. This position requires a full understanding and active participation in fulfilling the mission of Tarzana Treatment Centers, Inc. (TTC). It is expected that the employee demonstrates behavior consistent with the agency's core values. Services will be primarily provided in the field. </li><li>Administers Medications per the treating provider orders and assesses effectiveness, and monitors for side effects. </li><li>Completes pre-admission triage and determines medical/nursing appropriateness of admission. </li><li>Monitors patient's medication intake consistent with medication policies, practices and standards. </li><li>Communicates significant changes in patient's condition in a timely manner. </li><li>Documents in patient's chart nursing goals, interventions and clinical outcomes. </li><li>Functions as an active member of the treatment care planning team. </li><li>Responds to medical emergency situations. </li><li>Competently use equipment necessary for the delivery of patient care including but not limited to: glucometer, AED, EKG machine as appropriate, intermittent suction, oto/ophthalmoscope, bag/mask, oxygen. </li></ul> <p>10.Attends trainings as required for the position</p> <p>11.Performs other job-related functions as may be assigned by the Director of Nursing or Charge Nurse.</p> <p>EDUCATION/TRAINING/EXPERIENCE</p> <p>Graduate of an accredited LVN or LPT school or college and licensed to practice in California.
