<p>Description</p> <p>Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc. "The Heart of Our Community" provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care.</p> <p>Adolescent/Adult LICSW Summary</p> <p>Under the general supervision of the Integrated Behavioral Health Program Director, the LICSW's function is to provide assessment, consultation, case management, behavior health treatment and resource acquisition to both adult and adolescent patients/clients referred by the providers in conjunction with HealthFirst and the School-Based Health Center (SBHC).</p> <p>Schedule:</p> <p>Mon - Fri 8:30 am - 5:00 pm</p> <p>Required Qualifications</p> <ul> <li>Master's Degree in Social Work or Psychology from an accredited college or university. </li><li>Position requires a Massachusetts state license as a Licensed Independent Clinical Social Worker (LICSW) </li><li>At least 2 years post LICSW experience in Adult and Adolescent/Pediatric social work. </li><li>Community health or clinical background preferred. </li><li>Satisfactory completion of continuing education credits as mandated by any professional boards, licensing agencies, or credentialing organizations. </li><li>Bi-lingual preferred. </li><li>Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information. </li><li>Must have a valid driver's license and access to an insured vehicle as some transportation may be required. </li></ul> <p>Competencies</p> <ul> <li>High level of customer service and a positive approach to work with adolescent patients. Projects positive attitude about the department and the health center, and offers services to ensure positive experience. </li><li>Ability to maintain an organized work area and work collaboratively as a member of a team. </li><li>Must possess excellent oral and written communication skills and time management skills to effectively communicate with patients and other providers. </li><li>Ability to handle confidential information in a professional manner. </li><li>Works well under pressure to manage stressful situations. </li><li>Uses sound judgment and confidence in their practice. </li><li>Abides by the Code of Ethics outlined by the National Association of Social Workers. </li><li>Ability to multi- task. </li><li>Detail-oriented and accurate. </li><li>Maintain CPR certification. </li></ul> <p>All requirements are subject to possible modification to reasonably accommodate individuals with disabilities.
