<p>Developmental Pathways (DP) has an opportunity for a Senior Case Manager (SCM) - Coverage in our Systems Navigation Department. The SCM - Coverage will manage caseloads in coverage to ensure support is provided, aligning with rules and regulations using appropriate delegation. The SCM - Coverage will complete coverage tasks for individuals in coverage including required documentation related to screening and referral, eligibility determination and assessments, support plan development, PMPM activities, monitoring meetings, and other duties as assigned. The SCM - Coverage will also ensure the effectiveness of services delivered (as authorized by the service plan) and ensure identified outcomes are met, advocating for individual service needs. They will assist with training and ongoing support of Coverage Case Managers and others as needed. The SCM - Coverage will work with the Program Manager to determine needs amongst coverage and provide support with department needs as assigned.</p> <p>Our ideal Senior Case Manager is an empathetic, collaborative, and action-oriented employee with a desire to make an impact in the community and within our teams. To learn more about our commitment to inclusion and diversity, visit our website: https://www.dpcolo.org/about-us/inclusion-and-diversity/</p> <p>Requirements</p> <ul> <li> <p>A Bachelor's Degree,</p> </li><li> <p>OR Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities;</p> </li><li> <p>Or some combination of education and relevant experience appropriate to the requirements of the position</p> </li><li> <p>Must have demonstrated exemplary performance while working within service coordination or case management positions, including the following areas of performance: demonstrated initiative within the department or agency on projects, demonstrated leadership skills, adaptation to change, and problem-solving skills.</p> </li><li> <p>Valid Colorado motor vehicle license & proof of motor vehicle insurance</p> </li><li> <p>Ability to meet and maintain agency driving requirements</p> </li></ul> <p>Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.</p> <p>Preferred</p> <ul> <li> <p>Knowledge of rules and regulations for Health Care Policy and Financing and Colorado Department of Public Health and Environment</p> </li><li> <p>Two (2) years of experience in service coordination/case management at Developmental Pathways or similar agency</p> </li><li> <p>Familiar with Microsoft Office (Excel, Word, Outlook, etc.) and be able to use these and other required programs at an intermediate level</p> </li></ul> <p>Additional Responsibilities</p> <ul> <li> <p>Provide support with coverage management, research, assignments, etc.</p> </li><li> <p>Assist in completing hand-offs as it related to coverage</p> </li><li> <p>Serve as subject matter expert for applicable programs and services; serve as resources/point of contact for complex cases</p> </li><li> <p>Complete various assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools</p> </li><li> <p>Provide care coordination services and interventions by referring, educating, collaborating, and mediating with individual and external providers/agencies</p> </li><li> <p>Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines</p> </li><li> <p>Complete required documentation related to monitoring activities and other targeted case management activities as PMPM; evaluate need and effectiveness of services requested and delivered; advocate for individual service needs</p> </li><li> <p>Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary parties</p> </li><li> <p>Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family member</p> </li><li> <p>Maintain knowledge of state/federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs, and other community resources</p> </li></ul> <p>About Us</p> <p>Started in 1964, Developmental Pathways (DP) is a nonprofit agency serving more than 15,000 individuals with disabilities and their families in Arapahoe, Douglas, and Elbert counties.
