<p>The Public Health Assistant is a valuable member to our Physical Health team who performs clerical support for IBCCP Program as assigned. Responsible for determining program eligibility, scheduling appointments, data entry, translation for Spanish-speaking clients, filing, telephone contacts, correspondence duties, and community outreach. </p> <p>Scheduled Hours: 37.5 hours per week</p> <ul> <li>Determines eligibility for services by screening clients according to program requirements and guidelines. </li><li>Performs general office tasks. </li><li>Accurately performs data entry of program-related data and information in the Cornerstone and AS400 computer system. </li><li>Assists in educating the public, potential clients, and families, in community outreach activities. </li><li>Schedules clients for IBCCP Program appointments, follow-up appointments, and tracks results and recommendations under direct supervision. </li><li>Performs verbal language interpretation and written language translation as needed. </li><li>Develops and maintains a basic knowledge of services available at the Lake County Health Department and Community Health Center (LCHD/CHC). </li><li>Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities. </li><li>Develops and maintains a working knowledge of individual Emergency Preparedness roles. </li><li>Demonstrates adherence to National Patient Safety Goals. </li><li>Participates in quality improvement activities to assess, monitor and improve program services. </li><li>Participates in mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development. </li><li>Reports all unusual occurrences that may involve the safety and well being of the clients, patients and/or co-workers, directly to the supervisor and/or Safety Officer. </li><li>Assists and encourages clients/patients in identifying self-management goals. </li><li>Performs duties in a prioritized, organized and orderly manner to maximize clinic efficiency and productivity. </li><li>Maintains orderliness and cleanliness of clinic and work area. </li><li>Requires a High School Diploma or GED. </li><li>Requires one-year general office experience. </li><li>Data entry skills. </li><li>Basic knowledge of computers. </li><li>Ability to work on weekends holidays and overtime if necessary. </li><li>Ability to maintain and keep records. </li></ul> <p>As the largest human service provider in Lake County, we believe that services must be available without barriers.
