All while understanding that together we are even better.</p> <p>Schedule: Full Time</p> <p>Shift :Days 8- 4:30 pm</p> <p>The community you'll be caring for: AdventHealth Waterman - 4000 WATERMAN WAY, Tavares, 32778</p> <p>The role you 'll contribute :</p> <p>The Cancer Care Navigator provides guidance to patients through the cancer care continuum from diagnosis through survivorship, while being the central point of contact for a patient and coordinates all components involved in cancer care.</p> <p>T he value you'll bring to the team :</p> <ul> <li> <p>Assists with facilitating an evidence-based, multi-disciplinary, multi-specialty approach to cancer care across the care continuum.</p> </li><li> <p>Assists patients in understanding their diagnosis, treatment options, and available resources, including educating eligible patients about appropriate clinical research studies and technologies.</p> </li><li> <p>Serve as an essential link between patients and all other care providers.</p> </li><li> <p>Facilitate appointments for consults and support services within established service standards, including specialty physicians, lab services, diagnostic services, social work, and dietitians.</p> </li><li> <p>Works collaboratively with physicians and clinicians in navigating the cancer care patient (needs ranging from minimal to complex) from the diagnostic phase through treatment planning, medical management and surveillance/survivorship, assuring the patient has appropriate, timely medical and supportive services.</p> </li></ul> <p>We are an equal opportunity employer and do not tolerate discrimination based on race, color, creed, religion, national origin, sex, marital status, age or disability/handicap with respect to recruitment, selection, placement, promotion, wages, benefits and other terms and conditions of employment.</p>
