<p>This position is eligible for $5,000 retention bonus.</p> <p>Job Summary:</p> <p>The Providence Center Licensed Practical Nurse (LPN) ACT I Harmony Team provides integrated medical and behavioral health services as part of the Assertive Community Treatment (ACT I) team within TPC's Certified Community Behavioral Health Clinic (CCBHC) model. Individuals being treated on an ACT I team have complex Serious and Persistent Mental Illness (SPMI). This level of service is needed when an individuals care requires a multidisciplinary team approach to providing comprehensive treatment, support, and rehabilitation services in the community.</p> <p>The Nurse plays a vital role in assessing health needs, coordinating medical and psychiatric care, administering medications, and supporting overall wellness in community-based settings. This position serves adults with co-occurring substance use disorders, medical complexities, and unstable living arrangements. Experience working with individuals experiencing homelessness, addictions, or chronic health conditions is highly valued.</p> <p>Duties and Responsibilities:</p> <p>Triage and assess assigned clients on the team.</p> <p>Deliver person-centered, recovery-oriented clinical services as part of a multidisciplinary team providing comprehensive treatment, support, and rehabilitation to individuals with Serious and Persistent Mental Illness (SPMI).</p> <p>Ensure service intensity aligns with ACT I (High Intensity) standards, including a minimum of eight (8) or more client contacts per month, with at least four (4) contacts conducted in person OR four (4) hours qualifying services per client, per month in person. These contacts may be provided by any member of the multidisciplinary team to meet client needs.</p> <p>Assess and evaluate client health care data collected through interviewing, observation, record review and team input.</p> <p>Provides input and supports implementation of person-centered care plans as delegated by a Registered Nurse or licensed provider.</p> <p>Perform integrated care in accordance with appropriate medical protocol.</p> <p>Identify expected outcomes of care, individualized to the client, with the goal of improving clients overall health.</p> <p>Evaluate the clients response to the plan of care and update the plan based on clients response.</p> <p>Provide support to clients regarding daily living issues, conducting community and home visits as necessary.</p> <p>Instruct clients and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate.</p> <p>Educate clients on the various medications on the expected effects and side effects.</p> <p>Question any aspect of the clients treatment regimen which endangers the level of wellness.</p> <p>Administer medications as per prescriber orders (Injections and PO).</p> <p>Implement appropriately prescribed medication regimen while following the 6 (six) rights of medication administration.</p> <p>Monitor lab results and complete related documentation.</p> <p>Ensure that externally prescribed medications (from outside providers, e.g., the primary care provider) are accurately documented in EMR.</p> <p>Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications.</p> <p>Coordinate activities regarding consumer hospitalization.</p> <p>Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay.</p> <p>Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including prescriber.</p> <p>Collaborate with the staff members in providing care to clients.</p> <p>Perform recordkeeping in accordance with agency requirements.</p> <p>Complete annual nurses assessment forms. Licensed Practical Nurses may not complete initial assessments but may complete subsequent nursing assessments following the initial assessment completed by a Registered Nurse.</p> <p>Maintain cooperative relationships with clients, staff and the public.</p> <p>Assist clients with attending community (medical providers, clinics, etc.) appointments and transport clients in own vehicle when needed.</p> <p>Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy.</p> <p>Attend trainings, case presentations and conferences.</p> <p>Perform duties in accordance with departmental goals, objectives and standards.</p> <p>Serve on appropriate committees.</p> <p>Partake in training new hires and student Nurses.</p> <p>May be asked to share on-call with other nursing and program staff.</p> <p>Coordinate a variety of services.</p> <p>Provide support counseling, problem solving, contracts and limit setting.</p> <p>Establish attainable goals with the consumer.</p> <p>Involve clients, agencies and others in planning.</p> <p>Assist clients in deciding which areas they want to address.</p> <p>Assist clients with basic medical procedures and hygiene needs.</p> <p>Encourage clients to attain the highest possible levels of independence.</p> <p>Accept the values and lifestyles of clients.</p> <p>Treat clients with respect.</p> <p>Use a flexible approach with a diverse population.</p> <p>Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance users, as it relates to case management.</p> <p>Present verbal synopsis of actual cases as required.</p> <p>Make assessments and provide appropriate crisis interventions and risk management as needed.</p> <p>Assess clients' unmet needs (psychological, substance use, physical, housing, psychiatric).</p> <p>Identify, assess and respond to crisis situations.</p> <p>Perform in a calm and professional manner during crisis or emergency situations.</p> <p>Other duties as assigned.</p> <p>Requirements:</p> <p>Licensed Practical Nurse (LPN) or Compact License required.</p> <p>Associates degree (ASN) in Nursing required; Bachelor's degree (BSN) from accredited four-year College or University preferred.</p> <p>Experience in Human Services or Outpatient setting preferred.</p> <p>Experience working with psychiatric and substance use disorder patients also a plus.</p> <p>Bilingual Spanish preferred.</p> <p>Position requires driving.
