<p>Description:</p> <p>Job Summary:</p> <p>The Program Manager, Medical Re-Entry Program will oversee the design, development, and implementation of services tailored to support individuals re-entering the community after incarceration. This role involves close collaboration with the Behavioral Health Administrator, coordinating with various stakeholders, managing program staff, and ensuring that the program meets its goals in providing comprehensive support to participants. The Program Manager will also be responsible for evaluating program effectiveness and making data-driven improvements.</p> <p>Essential Responsibilities:</p> <p>Designs and develop re-entry programs that address the specific needs of participants. Collaborate with the Behavioral Health Administrator to implement program strategies and initiatives, ensuring alignment with organizational goals. Supervises and provides leadership to program staff, including hiring, education, training, and performance evaluations. Coordinates with community partners, government agencies, and other stakeholders to facilitate delivery service. Ensures compliance with all applicable laws, regulations, and organizational policies. Monitors program outcomes and metrics and prepares regular reports for the Behavioral Health Administrator. Advocates for program participants and work to remove barriers to successful re-entry. Stays up to date with best practices and emerging trends in re-entry programs/services. Conducts assessments of individuals to determine their needs and develop individualized re-entry plans. Manages program budget and resources effectively.</p> <p>Requirements:</p> <p>Education and/or Experience:</p> <ul> <li>Master of Public Health (MPH), Master of Science in Social Work (MSW) or Master of Science in Nursing (MSN) required. </li><li>Minimum of five (5) years of experience in a related field, including supervisory or management roles. </li><li>Peer Specialist certification strongly preferred. </li></ul> <p>Language Skills:</p> <p>English required.</p> <p>Bilingual English/Spanish preferred.</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.</p> <p>Knowledge/Skills/Abilities Required of the Position:</p> <p>Extensive knowledge of re-entry programming. Knowledge and understanding of the challenges faced by individuals re-entering the community after incarceration. Knowledge of relevant laws, regulations, and best practices in social work or nursing.</p> <p>Skill in oral and written communication is sufficient to exchange information. Skill is Leadership and Supervision. Excellent communication and interpersonal skills. Proficiency in data analysis and reporting.</p> <p>Ability to work independently and as part of a team. Ability to manage multiple tasks and projects simultaneously. Strong problem-solving and decision-making abilities.</p>
