The Assistant is responsible for making referrals to community care agencies and communicating these arrangements with the case manager and documenting in the patient's medical record. Communicates any relevant gap in providing care to a patient that is identified during the phone call.</p> </li><li> <p>Assist the manager in writing appeal letters for denials of in-patient admissions.</p> </li></ol>
