The position involves home visits, counseling, service coordination, crisis intervention, and advocacy to help consumers access medical, social, educational, vocational, and financial resources. Most duties are performed independently with general supervision.</p> <p>Primary Responsibilities:</p> <ul> <li> <p>Conduct home visits to provide supportive counseling and independent living assistance.</p> </li><li> <p>Help consumers modify behaviors, build insight, and develop coping skills.</p> </li><li> <p>Assess needs, gather medical/psychosocial information, and develop individualized service plans.</p> </li><li> <p>Maintain accurate case records, documentation, assessments, and quarterly reviews.</p> </li><li> <p>Provide 1:1 and group counseling using cognitive-behavioral and skills-training techniques.</p> </li><li> <p>Coordinate services with care managers, ICL staff, and external providers.</p> </li><li> <p>Assist consumers in accessing community resources, benefits, and vocational opportunities.</p> </li><li> <p>Perform crisis assessment and intervention; refer to appropriate services.</p> </li><li> <p>Accompany consumers to medical, social service, and government appointments.</p> </li><li> <p>Report incidents and complete incident documentation per policy.</p> </li><li> <p>Conduct safety and cleanliness inspections of consumer residences.</p> </li><li> <p>Plan and facilitate recreational or social activities.</p> </li><li> <p>Participate in meetings, supervision, and required training.</p> </li><li> <p>Advocate for consumer needs within ICL and with external providers.</p> </li><li> <p>Support discharge planning and goal achievement.</p> </li><li> <p>Perform other related duties as assigned.</p> </li></ul>
