This includes, but is not limited to: </li><li>Patient referrals </li><li>MD office visit notes </li><li>H&P </li><li>Sharing of Plan of Care with other entities </li><li>Tracking of supplemental physician orders and plans of care </li><li>Input of basic non-clinical information into the EMR </li><li>Performs other related duties as assigned </li></ul> <p>Qualifications</p> <p>What You Will Bring:</p> <ul> <li>Ability to exercise independent judgment </li><li>Ability to demonstrate above accuracy attention to detail to ensure accuracy </li><li>Ability to read and interpret medical record data for chart audit </li></ul> <p>Education and Experience:</p> <ul> <li>High school diploma or GED required </li><li>CMA/QMA preferred </li><li>One (1) year experience in care coordination preferred </li><li>Knowledge of service regulations and process </li><li>Phone/computer experience </li></ul> <p>The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification. This position is responsible for organizing patient care and treatments by including all members of the patient's care team. This role assists the RN Case Managers in duties such as care coordination, order tracking, and filing.</p>
