<p>Case Manager - Behavioral Health and Social Services Department: Behavioral Health and Social Services FLSA Status: Non-Exempt Job Grade: 5 Version Date: June 2022</p> <p>General Responsibilities: We are seeking a dedicated Case Manager to provide essential case management services, financial resources counseling, crisis intervention, and supportive counseling. The ideal candidate will support and advocate for patients to reduce barriers to health care.</p> <p>Qualifications:</p> <p>Required Qualifications:</p> <ul> <li>Bachelor's Degree in Social Work or a related field </li><li>Excellent written and oral communication skills </li><li>One year of social work case management experience with high-risk families and individuals </li></ul> <p>Significant Job Functions:</p> <p>Clinical:</p> <ul> <li>Function as part of the clinical team, accepting referrals from medical providers and other agencies. </li><li>Provide psychosocial assessments and determine the need for case management services. </li><li>Offer necessary referrals for services such as mental health and substance abuse counseling, DCYF, HIV/AIDS case management, Health Care for the Homeless, Family Planning, and others as appropriate. </li><li>Develop treatment plans with clients, setting goals and objectives. </li><li>Participate in clinical supervision, presenting cases for review. </li><li>Develop, implement, and coordinate psychoeducational programs targeting various public health prevention initiatives. </li><li>Provide prenatal assessments, counseling, and referrals to community agencies. </li></ul> <p>Organizational:</p> <ul> <li>Participate in community committees and initiatives as appropriate. </li><li>Participate in THC committees as needed. </li></ul> <p>Physical Effort/Environment:</p> <ul> <li>This position is performed onsite at a Thundermist location. </li><li>Moderate physical activity is required, including mobility to meet with patients throughout the health center and occasional home visits. </li><li>Writing ability is necessary for patient records. Computer skills are also required. </li></ul> <p>Work Schedule Demands:</p> <ul> <li>As scheduled. Some evenings may be required, with flexibility needed for patient crises. </li></ul> <p>Communication Skills:</p> <ul> <li>Requires excellent oral and written communication skills at all levels of client and staff interaction. </li><li>Must document interactions clearly and concisely in medical and social service records. </li><li>Ability to communicate effectively as part of the clinical team. </li></ul> <p>Confidentiality of Information: Thundermist is committed to securing the privacy and confidentiality of protected health information under the Health Insurance Portability and Accountability Act.
