<p>Title: Perinatal Outreach Nurse</p> <p>Unit: Help Me Grow Brighter Futures</p> <p>Hours: 8a-4:30p</p> <p>Facility: MVH</p> <p>Position Summary:</p> <p>The Perinatal Outreach Nurse (PON) functions in a multifaceted role that demonstrates professionalism and promotes excellence in the practice of Nursing. The PON is responsible for maintaining high standards in clinical practice/continuum of care, professional practice development, teamwork, customer relations, and research/performance improvement in delivering case management and support services for a designated caseload of patients. Home visits will be made with this caseload of pregnant/parenting women and their children under the program research protocols. The PON will also interact with appropriate community agencies in accordance with guidelines. The PON is accountable for compliance with the Ohio Nurse Practice Act and all applicable regulatory standards. Policies, procedures and protocols of the Human Resources Department, GDAHA, Help Me Grow, and the Nurse-Family Partnership research project are followed.</p> <p>The PON reports to the Home Visit Coordinator for the Nurse Family Partnership. The Nurse is accountable for compliance with all applicable regulatory standards, policies, procedures, and protocols of the Human Resources Department, GDAHA, the Help Me Grow Program and any funding sources for the work of the department. The Nurse will comply with Indicated data base and data entry requirements.</p> <p>Essential Duties & Functions:</p> <ol> <li> <p>Collaborate with the Home Visit Coordinator to identify learning needs and formulate self -development goals/developmental plan.</p> </li><li> <p>Meet with the Nurse Home Visit Coordinator for supervision - both administrative and reflective.</p> </li><li> <p>Create a positive work environment that promotes productivity.</p> </li><li> <p>Receptive to new ideas and methods and willing to adapt to change.</p> </li><li> <p>Documents work schedule in Outlook including client visits and other related meetings.</p> </li><li> <p>Provide home visits to 25-30 pregnant and parenting mothers using the Nurse Family Partnership model at 85% productivity or better.</p> </li><li> <p>Average time to first visit is as soon as possible and within 14 days.</p> </li><li> <p>Documentation is completed within 3 working days.</p> </li><li> <p>Screenings, tools and family plans are completed on time 95% of the time.</p> </li><li> <p>Time track is timely and reflective of daily work.</p> </li><li> <p>Attend weekly staff meetings.</p> </li><li> <p>Demonstrates willingness to accept and efficiently process new families.</p> </li><li> <p>Clients are exited in a timely fashion.</p> </li><li> <p>Assist with orientation of new staff.</p> </li><li> <p>Promote a seamless continuum of care between all aspects of care and community resources.</p> </li><li> <p>Adhere to policies concerning confidentiality and consumer relations.</p> </li></ol> <p>Contribute to the efficient, orderly operations of the department</p> <p>Education</p> <p>Minimum Level of Education Required: Bachelor's degree</p> <p>Additional requirements:</p> <p>Type of degree: Bachelor's degree</p> <p>Area of study or major: Nursing</p> <p>Preferred educational qualifications:</p> <p>Position specific testing requirement: Must undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio central registry on child abuse and neglect.</p> <p>Licensure/Certification/Registration: Licensed Registered Nurse</p> <p>Experience:</p> <p>Minimum Level of Experience Required: 1 - 3 years of job-related experience</p> <p>Prior job title or occupational experience: Prefer experience in a variety of settings such as Home visiting, OB/Gyn, Pediatrics, Adolescents, or Mental Health.</p> <p>Preferred experience: At least 1 year working with pregnant women, infants, and children.</p> <p>Other experience requirements: Complete orientation per the Nurse Family Partnership and Help Me Grow guidelines.</p> <p>Knowledge/Skills:</p> <ul> <li>Must possess and display effective verbal, written and interpersonal communication skills. </li><li>Organized and self-directed. </li><li>Valid driver's license and insured safe automobile. </li><li>Non-smoker </li></ul>
