City of Hope's growing national system includes its Los Angeles campus, a network of clinical care locations across Southern California, a new cancer center in Orange County, California, and cancer treatment centers and outpatient facilities in the Atlanta, Chicago and Phoenix areas.</p> <p>The successful candidate:</p> <p>Under the supervision of the Director/Manager of Pharmacy, a clinical pharmacist is responsible for all services, both distributive and clinical, within the ambulatory infusion setting and inpatient units. Clinical pharmacists are essential to our care model within COH. They are accountable for working directly with multidisciplinary teams in the Medical Oncology and Surgical Oncology units to provide high-quality evidence-based care as well and discharge counseling services. Pharmacists practice through hospital policies, practice agreements and clinical guidelines to manage disease states and chemotherapy toxicities, perform pharmacokinetic services and ensure safe, cost-effective, evidence-based use of medications. In addition to inpatient rounding shifts, the pharmacist is also responsible for staffing in our ambulatory infusion center. Responsibilities include chemotherapy verification, medication dispensing, drug information and close collaboration with oncologists, midlevel providers and infusion center nurses</p> <p>Position Qualifications:</p> <p>Minimum Education:</p> <p>Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education</p> <p>Required Courses/Training:</p> <p>Completion of pharmacy residency program or equivalent hospital pharmacy experience preferred.</p> <p>Req.
