<p>Job Title: Licensed Practical Nurse</p> <p>Job Description</p> <p>We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. This role focuses on providing routine nursing care and offers support in managing high-risk and chronic care, along with referral specialist support. Our healthcare facility is a Federally Qualified Health Center, providing primary healthcare services to children and adults in Greenwich and Stamford, with a particular focus on populations residing in or near public housing.</p> <p>Responsibilities</p> <ul> <li>Provide routine nursing care to patients in a primary care setting. </li><li>Support high-risk and chronic care management, including coordinating care for elderly and disabled patients. </li><li>Manage care for patients with chronic conditions such as Diabetes, HIV, Hypertension, and Obesity. </li><li>Assist with referral processes and care coordination. </li><li>Travel between Stamford and Greenwich as required for patient care. </li><li>Support women's health initiatives and conduct medical assessments. </li><li>Monitor and record vital signs accurately. </li></ul> <p>Essential Skills</p> <ul> <li>Current LPN licensure in Connecticut. </li><li>Minimum of 3 years of experience working as an LPN. </li><li>Experience in high-risk or chronic care management. </li><li>Proficiency with electronic health records (EHR). </li><li>Excellent communication and interpersonal skills. </li><li>Ability to work collaboratively within a multidisciplinary team. </li><li>Bilingual proficiency in Spanish or Haitian Creole. </li></ul> <p>Additional Skills & Qualifications</p> <ul> <li>Minimum of 2 years of experience in a primary care or outpatient setting preferred. </li><li>Knowledge of population health, care coordination, and referral processes. </li><li>Strong clinical skills in primary care and chronic disease management. </li><li>High attention to detail and ability to manage multiple responsibilities. </li><li>Ability to work independently and as part of a team. </li><li>Strong organizational and time management skills. </li><li>Compassionate, patient-centered approach to care. </li></ul> <p>Work Environment</p> <p>The work environment is a community health center with flexible scheduling. The office operates Monday through Friday from 8 am to 5 pm, and caters to patients of all ages, from infants to geriatrics. Training will be provided, ranging from 2 to 4 weeks, depending on the candidate's ability to learn quickly. Training includes onboarding, orientation, and shadowing. Physical requirements include the ability to stand and walk for up to 8 hours a day, stoop, kneel, crouch, extend without restrictions, and lift a minimum of 50 lbs.
