<p>Responsibilities:</p> <ul> <li>The LPN is responsible for administration of all medications and treatments during their shift, according to the established medical and nursing care plan. Works under the direction of the Head/Charge Nurse to provide optimum nursing care to all residents on the unit. </li><li>Continuously evaluates behavior, symptoms, reactions and progress of residents and discusses findings with Nurse Manager/Charge Nurse. </li><li>Administers and documents all medications according to established policies and procedures, physician's orders, and Department of Health regulations. </li><li>Performs all treatments according to physician's specific order and established policies and procedures. </li><li>Discusses the need to discontinue or change a medication of treatment with the Nurse Manager/ Charge Nurse. </li><li>Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy. </li></ul> <p>Qualifications:</p> <ul> <li>Current NY State Licensed Practical Nurse </li></ul> <p>Education:</p> <ul> <li>Accredited LPN School </li></ul>
