This role is designed for an educator who excels at both supporting students within the general education environment and delivering targeted, individualized instruction. As a Case Manager, you will ensure students receive high-quality support through a hybrid model of inclusion-based services and specialized individual or small-group instruction tailored to meet their unique IEP goals.</p> <p>MAJOR DUTIES & RESPONSIBILITIES:</p> <ul> <li> <p>Case Management: Lead the development, implementation, and monitoring of Individualized Education Programs (IEPs) for a designated caseload.</p> </li><li> <p>Inclusion Support: Partner with general education teachers to provide "push-in" support, facilitating modifications and accommodations within the regular classroom.</p> </li><li> <p>Individualized Instruction: Design and deliver specialized, one-on-one or small-group instruction to address specific skill gaps and academic needs outside of the general education setting as required.</p> </li><li> <p>Collaboration: Work closely with the middle school team to differentiate curriculum and ensure students have access to the general education environment while receiving the specific interventions they need.</p> </li><li> <p>Compliance & Meetings: Facilitate PPT (Planning and Placement Team) meetings and maintain rigorous documentation in accordance with state and federal regulations.</p> </li><li> <p>Progress Monitoring: Use data-driven methods to track student growth and adjust instructional strategies to ensure mastery of IEP goals.</p> </li></ul> <p>MINIMUM QUALIFICATIONS:</p> <ul> <li>Certification: Valid CT Teaching Certificate with endorsement Comprehensive Special Education (165, 065, 265) or documented eligibility for certification. </li><li>Experience with middle school-aged students and a variety of service delivery models (Inclusion and Resource) preferred. </li><li>Proven ability to write and implement effective IEPs and collect meaningful progress data. </li><li>High degree of professionalism and a commitment to maintaining student confidentiality. </li><li>Background Check: Ability to pass a comprehensive background check and meet all district employment requirements </li></ul> <p>RATE OF PAY: $56,198 - $100,761 per year (determined by contractual agreement)</p> <p>APPLICATION PROCEDURE:</p> <p>EXTERNAL APPLICANTS: Please submit your application through our website: www.putnamschoolsct.org</p> <p>INTERNAL APPLICANTS: Please submit a letter of interest and updated resume to Gregory Keith, Principal and Rebecca Lopes, Director of Student Services</p> <p>START DATE: 2026-2027 School Year</p> <p>APPLICATION CLOSING DATE: Open until filled</p> <p>Equal Opportunity Employer</p>
