<p>Job Summary:</p> <p>The care management position is responsible for assisting patients and families in navigating their healthcare. Their primary focus is high-risk patient populations within primary care pediatrics. Each role will encompass patient care as well as being the lead coordinator for all services related to the assigned patients; working closely with multiple agencies, specialty services, and providers. This role will be responsible for the patient's individual needs as they relate to age, physical, psychosocial, cultural, and education.</p> <p>Minimum Qualifications:</p> <p>R.N. NYS license or eligible, Bachelor's degree in Nursing or healthcare related field, plus a minimum of three (3) years RN nursing experience required OR equivalent combinations of education and experience. CPR-BLS is required at the time of appointment.</p> <p>Preferred Qualifications:</p> <p>Master's degree in Nursing or a healthcare-related field.
