<p>The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a Social Worker to join our team! This position is 30 - 37.5 hours per week, Monday - Friday, 8:30 AM - 3:00 or 4:30PM, including alternating Saturdays 8:30 AM - 3:30 PM.</p> <p>Job Responsibilities:</p> <ul> <li>Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program. </li><li>Coordinate and facilitate care planning and completion of all documentation </li><li>Facilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends. </li></ul> <p>Job Qualifications:</p> <ul> <li>MSW from an accredited School of Social Work. </li><li>Valid New York State License in Social Work. (LMSW) </li><li>Five (5) years dementia care experience </li><li>Bi-lingual (English/Spanish) a plus </li><li>Certified in Dementia Care a plus </li><li>Experience in adult day care center preferable. </li><li>Solid written, communication and organizational skills </li></ul>
