<p>Job Responsibilities</p> <p>The Agency's mission is to be boldly innovative in improving the health and quality of life for South Carolinians.</p> <p>This position is located in Community Long Term Care (CLTC), Sumter County.</p> <p>This is an in-office role and not a telecommute or remote position.</p> <p>Are you the One? We are looking for a Social Worker (Case Manager III), who works under general supervision of CLTC Area Administrator. Serves as Case Manager Il. Assists in the implementation of a system of case management for Medicaid eligible applicants and enrolled participants. Assists other CLTC offices with essential job functions according to program needs. Trains workers and performs assigned quality assurance activities and compliance activities as directed. Travel to location of assigned cases necessary for this position. Maintains a thorough working knowledge of all aspects of CLTC and related systems: programs, policies, and procedures, including but not limited to waivers, Phoenix and Care Call.</p> <ul> <li>Completes waiver enrollment activities </li><li>Provides case review related to assessment and service planning </li><li>Conducts service Prior Approval process </li><li>Performs Quality Assurance and Compliance Activities </li><li>Provides case management services </li><li>Promotes Collaboration and Effective Communication </li><li>Performs emergency preparedness duties or activities as required </li></ul> <p>The South Carolina Department of Health and Human Services offers an exceptional benefits package for FTE and TGE positions that includes:</p> <ul> <li>Health, Dental, Vision, Long Term Disability, and Life Insurance for Employee, Spouse, and Children. </li><li>15 days annual (vacation) leave per year. </li><li>15 days sick leave per year. </li><li>13 paid holidays. </li><li>State Retirement Plan and Deferred Compensation Programs. </li></ul> <p>Minimum and Additional Requirements</p> <p>A Bachelor's Degree in social work or social welfare program accredited by the Council on Social Work Education, or from a social work or social welfare program offered by a regionally accredited institution, plus one (l) year of experience as a professional social worker.</p> <p>MANDATORY SPECIAL REQUIREMENT: Must have current licensure by the SC Board of Social Work Examiners.</p> <p>Additional Requirements:</p> <ul> <li>Valid driver's license. </li><li>Lifting requirements: 20 lbs. </li><li>Requires holder to drive routinely </li><li>In-office Role </li></ul> <p>Preferred Qualifications</p> <ul> <li>Community Long Term Care experience and gerontology preparation. </li><li>Knowledge of criteria for level of care determination. </li><li>Knowledge of federal and state Medicaid regulations. </li><li>Understanding and interest in working with the elderly and disabled. </li><li>Ability to utilize and accept supervision and consultation. </li><li>Ability to train new staff and contract providers. </li><li>Ability to communicate effectively both verbally and in writing. </li><li>Personal computer skills.
