<p>Position Summary Clinical Pharmacist - Ambulatory Pharmacy Services Location: Ocoee Area Schedule: Full-Time, Variable M-F 9-5:30 pm On call every 6 weeks - able to answer calls About Orlando Health Scripts Specialty Pharmacy Orlando Health Scripts serves patients with chronic conditions requiring specialty medications. Position Overview The Clinical Pharmacist is responsible for delivering comprehensive medication management services, including: Targeted medication reviews Therapeutic drug monitoring Patient education and counseling Drug therapy consultations Medication reconciliation Clinical in-services and education programs This role supports the Ambulatory Pharmacy Services and Patient Management Program to ensure safe, effective, and patient-centered care. Analyze and verify accuracy, appropriateness, and safety of prescribed therapy. Conduct comprehensive and targeted medication reviews, disease state therapy management counseling, and collaborate with healthcare providers for pharmaceutical interventions. Review patient medication regimens for appropriateness, including dose, route, method of administration, therapeutic duplication, contraindications, drug interactions, cost-effective drug selection, and therapeutic outcomes. Interview patients, monitor medication effects, make dosing adjustments under approved protocols, request lab tests, and confer with medical team members as needed. Perform medication reconciliation to prevent, identify, and resolve drug-related problems, ensuring continuity of care and improved patient adherence. Monitor allergies, lab values, side effects, adverse drug reactions, and therapy interactions; report drug errors and compliance issues per pharmacy procedures. Plan, organize, execute, and monitor pharmaceutical care plans using critical thinking and problem-solving skills. Document in appropriate systems and forms consistent with guidelines, standards of care, and quality improvement programs. Qualifications Licensure/Certification Must be licensed by the Florida Board of Pharmacy (renewed every two years). License must be current and unrestricted. Experience & Competencies Minimum of 2 years of experience in clinical practice or specialty pharmacy OR completion of an ASHP-accredited residency. Experience in Ambulatory Pharmacy or Specialty Pharmacy highly preferred. Proficiency in Microsoft Office products (Word, Excel, PowerPoint) and other presentation software.</p> <p>Licensure/Certification Must be licensed by the Florida Board of Pharmacy (renewed every two years).
