Youll help shape decisions that elevate both patient outcomes and the future of nursing.</p> <p>Job Summary and Qualifications</p> <p>What you will do in this role:</p> <ul> <li>Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care. </li><li>Establishes effective communication with physician offices, hospital departments, staff, and rehabilitation services to facilitate a smooth continuum of care for all bariatric patients and families. </li><li>Coordinates care with case management to anticipate high-risk patient issues and assist in implementing an efficient discharge/rehab plan for bariatric patients. </li><li>Develops education for bariatric staff including treatment protocols and bariatric inservices. </li><li>Coordinates postoperative follow-up calls to patients and responds to patient calls addressing any questions and concerns. </li><li>Assumes responsibility for coordinating components for Disease Specific Certification programs. Works with multidisciplinary team members on performance improvement initiatives. </li><li>Coordinates and provides bariatric education classes for patients scheduled for a surgical procedure. Completes nurse leader rounding. </li><li>Participates and coordinates (with the marketing department) community bariatric educational programs presented by physicians. </li><li>Works cooperatively and communicates effectively with all team members to ensure coordination of care and documentation of services provided. </li><li>Pursues professional growth and development. </li></ul> <p>What qualifications you will need:</p> <ul> <li>Basic Cardiac Life Support must be obtained within 30 days of employment start date </li><li>(RN) Registered Nurse </li><li>Registered Nurse Diploma </li><li>2 years of acute care/surgical services experience required </li><li>Bariatrics/Nurse Navigator experience preferred </li></ul> <p>Benefits</p> <p>Parham Doctors Hospital, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
