Finally, being patient-centered, we prioritize the needs and well-being of those we serve, striving to exceed expectations and make a meaningful difference in their lives.</p> <p>A day in the life of this role:</p> <p>Reporting to the Medical Director, the Primary Care Nurse Practitioner (NP) / Physician Assistant (PA) provides comprehensive and continuing care to patients requiring routine and emergency treatment at health centers or other locations within OCHS's scope of practice. The Primary Care NP/PA will have an assigned panel of patients. They will assess, diagnose, and treat episodic illnesses as well as manage chronic health problems of adult and pediatric patients within their panel. They will also provide coverage and care for patients under the care of their colleagues as needed to improve access to care and to ensure a positive patient experience and positive medical outcomes.</p> <p>Summary of duties:</p> <ul> <li>Serve as part of a Patient Centered Medical Home team to ensure consistent coverage and access for all OCHS patients to Primary Care and same day medical services when appropriate. </li><li>Have a passion for and/or experience working as a team-based care model. </li><li>Supervise other health care professionals such as staff registered nurses, medical assistants, phlebotomists and other allied health care staff, as appropriate, to ensure the proper delivery of medical care to patients. </li><li>Collaborate with the practice manager and the medical director regarding daily/weekly schedule and time off requests to ensure adequate patient coverage. </li><li>Medically manage or support the care of emergency, urgent, and other walk-in patients and cooperate with site staff in managing such needs. </li><li>Diagnose illness or health needs based on analysis and evaluation of clinical data collected; plan and provide treatment according to standard of care/protocols and/or consultation with supervising physician and/or Medical Director as needed. </li><li>Perform physical examinations, order laboratory tests and analyze results to evaluate acute/chronic complaints. </li><li>Perform in office procedures/minor surgeries as credentialed and deemed competent, with support of supervising physician and/or medical director as warranted </li><li>Keep accurate, complete, legible and timely medical and other records and files as required by OCHS' policy. </li><li>Participate in peer review and patient care audits for quality assurance and utilization review to ensure an excellent standard of care. </li><li>Participate in On Call duties as assigned. </li><li>Maintain knowledge of current developments in specialty to enhance professional expertise including maintenance of Board Certification and completion of continuing medical education credits as needed to maintain licensure. </li><li>Maintain the highest standard of professionalism toward colleagues, patients and all OCHS stakeholders. </li><li>Participate in clinician and staff meetings as set by OCHS' leadership and management teams </li><li>Perform other related duties as reasonably required or assigned. </li><li>Adhere to OCHS' policies and procedure </li></ul> <p>Work environment:</p> <p>?
