Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services.</p> <p>Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity's Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area.</p> <p>We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.</p> <p>AMAZING Benefits!</p> <p>At Intercommunity, we believe our benefits should make a difference to you, to your family and to the life you lead outside of work. Benefits take effect on the first day of the month following the hire date.</p> <p>Summary:</p> <p>The Clinician 2 provides therapeutic (group, individual and case management) services to the clients of the program while ensuring the safe operation of the facility. The Clinician 2 is also expected to assume the daily operations of the program in the absence of the Program Manager or VP of Community Based Services.</p> <ul> <li>Completes psychosocial histories and assessments, treatment plans, treatment diagnosis, authorizations and discharge summaries </li><li>Provide ongoing individual and group counseling and appropriate documentation </li><li>Works with a multidisciplinary team or residential team to co- coordinate treatment and discharge planning. </li><li>Arranges further treatment as appropriate </li><li>Transports clients when necessary to appointments and placements. </li><li>Work under the supervision of the Program Manager and VP of Community Based Services </li><li>Attends in-service training and maintain their clinical licensure </li><li>Performs other related duties as assigned by supervisor </li><li>Assumes the responsibilities of the Program Manager in his/her absence </li><li>Assist the Program Director and VP of Community Based Services with staff development, training and client interviews as needed. </li><li>Helps train volunteers, student interns, counselors, and residential support specialists </li><li>Participates in in-house trainings </li></ul> <p>Schedule:</p> <p>Monday - Thursday, 2 pm to 10:30 pm</p> <p>Friday - 10 am - 6:30 pm</p> <ul> <li>All agency staff are required to attend all mandatory department/agency meetings and trainings </li></ul> <p>Requirements</p> <p>Education &/Or Experience:</p> <p>A Master's degree, and two (2) years relevant experience and possess a LCSW or LPC.
