For more information on how to join our dynamic team, please visit our website at www.swhealthcaresystem.com.</p> <p>Job Summary:</p> <p>Southwest Healthcare is seeking a Full-Time Medical Records Analyst at our Temecula, CA location. This position will collaborate with staff across the Region and is responsible for auditing medical record charts and accurately reviewing them according to established Medical Staff Bylaws, Medical Staff Rules and Regulations and Federal and State Statutes. Duties include but not limited to:</p> <ul> <li>Consistently accounts for 100% of all charts analyzed on the discharge lists within 24-hours of discharge. </li><li>Compiles accurate statistical data on chart audits concurrently and/or after discharge for reporting purposes using the latest performance improvement tools/methods. </li><li>Follow-up with each provider with a phone call/meeting when an electronic communication is ineffective to ensure the provider has an understanding of a process/procedure to meet compliance. </li><li>Prepae Medical Records and scan documents according to the department procededures. </li><li>Communicates any hand-off information for your primary and secondary function(s) to the responsible staff member to ensure continuous workflow when you have knowledge of upcoming absences. </li><li>Perform quality control functions on all objects scanned comparing the paper record to the scanning system ensuring quantitative and qualitative documentation at a 95% accuracy rate. </li><li>Track prepping and scanning errors discovered through the quality control process to help with performance improvement. </li><li>Performs corrections within the current EHR system or notify appropriate staff member for correction when discovered. </li><li>Daily document all productivity data within your individual spreadsheets. </li><li>Performs other duties as assigned. </li></ul> <p>Qualifications</p> <p>EDUCATION/TRAINING/ EXPERIENCE:</p> <ul> <li>High School Graduate or equivalent required. </li><li>Associate's degree from an accredited College or University in Medical Records Science preferred. </li><li>Completion of an introductory course of Medical Terminology required. </li><li>Two (2) to Three (3) years of experience in Medical Records required, acute care experience preferred. Equivalent education in lieu of experience will be considered. </li><li>Prior knowledge of State and Federal release of information laws preferred. </li></ul> <p>CERTIFICATIONS/LICENSES:</p> <ul> <li>Must have valid California Driver's License and reliable transportation. </li></ul> <p>OTHER SKILLS AND ABILITIES:</p> <ul> <li>Demonstrates knowledge and ensures compliance with The Joint Commission and Title 22 standards and guidelines. </li><li>Demonstrates compliance with Healthcare policies and procedures at all times. </li><li>Ability to set priorities and appropriately organize workload and complete assignments in a timely manner. </li><li>Demonstrates ability to relate to clinical personnel and medical staff, as well as ability to interact well with the public. </li><li>Must have knowledge of PC and applications. </li><li>Must possess an ability to use and access reference material and make decisions upholding Medical Staff Bylaws, Rules and Regulations, The Joint Commission standards, Federal and State (Title 22) statute, as it relates to medical records completion. </li><li>Demonstrates the ability to adhere to all Health Insurance Portability and Accountability Act (HIPAA), Federal and State statute, as it related to proper and improper releases. </li><li>Demonstrates a high level of awareness in protecting patient P.H.I., including double check using two patient identifiers for verification of documentation that may be given to outside agencies or patients. </li><li>Demonstrates compliance with Healthcare policies and procedures at all times. </li><li>Ability to set priorities and appropriately prioritize workload and complete assignments in a timely manner. </li><li>Demonstrates excellent organizational skills and detail oriented. </li><li>Demonstrates ability to relate to clinical personnel and medical staff, as well as ability to interact well with the public. </li><li>Demonstrates effective communication with all customers (i.e. medical staff, hospital staff, patients, etc.) regardless of communication method.
