<p>Case Management Social Worker - Social Service - 40 Hours -Days</p> <p>Eligible for Full-Time Benefits</p> <p>Shift: Days 7:30 am to 4:00 pm, occasional weekend and holidays required.</p> <p>GENERAL SUMMARY:</p> <p>Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.</p> <p>Responsibilities:</p> <ul> <li>Creates plan for care across the continuum, integrating patient/family preferences and values. </li><li>Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs. </li><li>Advocates for resources and removal of barriers. </li><li>Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. </li><li>Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care. </li></ul> <p>EDUCATION/EXPERIENCE REQUIRED:</p> <ul> <li>Bachelor's degree required. </li></ul> <p>EDUCATION/EXPERIENCE PREFERRED:</p> <ul> <li>Master's degree preferred. </li><li>Acute Care Social Work experience preferred. </li></ul> <p>CERTIFICATION/LICENCES REQUIRED:</p> <ul> <li>One or more of the following required: </li><li>Counselor credentialed from the Michigan Board of Counseling obtained prior to hire date or job transfer date. </li><li>Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date.
