Apply now and let's soar together at Blue Skies of Texas! As a Hospice RN Case Manager, you will be responsible for providing skilled professional nursing care to the patient and family. The incumbent oversees quality of care delivery by all disciplines and programs, assures delivery of optimal care safely and effectively, and identifies both existing and potential problems that impact the quality of patient care. Also ensures that provision of the highest quality of care to residents receiving Hospice services. This role is critical in directing, assessing, planning, implementing, and overseeing the care provided to patients. The incumbent occupies a position of confidence and trust and must maintain confidentiality in accordance with Health Insurance Portability and Accountability Act regulations. What you will do in this role: Coordinates admissions into The Mission and The Vista. Coordinates accurate and timely completion of admission agreements and collects all necessary forms and documents, verifies insurance coverage and Medicare coverage, and identification of responsible party information required for admission of resident. Assures discharges and transitions are routed to appropriate level of care to ensure appropriate discharge planning. Responds to all inquiries and interviews of potential residents, family members and discharge planners to collect accurate data on admissions inquiries. Conducts tours for potential referrals sources as needed. Communicates with all appropriate departments in a timely manner, regarding bed availability.</p> <p>Qualifications:</p> <ul> <li>Required: Registered Nurse license in good standing with the state of Texas or a compact state allowing practice in Texas. </li><li>Required: Current certifications in: Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED). </li><li>Required: Valid Texas driver's license with a good driving record; must remain insurable under Blue Skies carriers. </li><li>At least 1-3 years in clinical nursing for Hospice care with knowledge of acute and chronic disease management </li><li>Working knowledge of computer skills, including Microsoft Office (Word, Excel, PowerPoint). </li><li>Training provided to ensure safe and proper use of job-related equipment. </li><li>Preferred: A bachelor's degree in Nursing, as well as at least one year of clinical nursing experience-preferably in hospice-with a focus in adult or geriatric nursing, acute and chronic disease management, patient teaching, and phlebotomy and intravenous skills. </li><li>Preferred: Certification in Hospice. </li></ul> <p>WORK SCHEDULE: Core work hours are as needed from 7:30 am to 4:30 pm, Monday through Friday; however, hours are subject to change depending on patient needs. Special projects or assignments may require additional hours to include evenings, weekends, and holidays. Rotating on-call duty is a requirement of this job.</p> <p>WORK LOCATION: Most work will take place at Freedom House; however, travel to the East and West campuses and in the greater San Antonio area is an essential part of this job.</p> <p>In 1970, the first of our two campuses opened in San Antonio.
